Nearly 30 years after introducing evaluation and management codes, the American Medical Association (AMA) made significant changes to this code set for the first time with its 2021 CPT® code update, revising coding guidelines for CPT outpatient evaluation and management services.

The AMA and the Centers for Medicare & Medicaid Services have completed a major overhaul of evaluation and management (E/M) office visit documentation and coding. These landmark changes to E/M office visit coding went into effect on January 1, 2021.

“To get the full benefit of the burden relief from the E/M office visit changes, health care organizations need to understand and be ready to use the revised CPT codes and guidelines by Jan. 1, 2021,” said AMA President Susan R. Bailey, M.D. These changes announced in November 2019 are designed to address administrative burden in clinical practice, and impact nearly every medical specialty. Understanding when these changes are coming, and their benefits and impacts, is essential for all members of the care team. This module provides key information about these landmark revisions. Overall, the 2021 CPT code set has 328 editorial changes, including 206 new codes, 54 deletions, and 69 revisions. The majority of new codes in 2021 (63 percent) are involved in new technology services described in Category III CPT codes and the continued expansion of the PLA section of the CPT code set, AMA reported.

The changes below relate only to new and established patient visits in 2021, codes 99202—99215. These changes are in the 2021 CPT book.
